— WHAT IS HAZOP —

HAZOP Study —
What It Is & Why It's Required

A HAZOP (Hazard and Operability Study) is a structured, systematic examination of a process design to identify hazards and operability problems. Conducted by a multi-disciplinary team using guide words applied to process parameters, HAZOP is the most widely used Process Hazard Analysis (PHA) methodology for chemical and process industries worldwide — and is mandated by MSIHC Rules 1989 for Gujarat industries handling hazardous chemicals above threshold quantities.

MSIHC Rules 1989 — MHI Mandatory

Major Hazard Installations (MHI) handling Schedule 1 chemicals above threshold quantities must conduct HAZOP or equivalent PHA. DISH Gujarat inspects MHI compliance including current HAZOP documentation.

MoEFCC EC Compliance Requirement

MoEFCC Environmental Clearance conditions for chemical/petrochemical projects typically require PHA/HAZOP as a pre-commissioning condition. EC renewal may require HAZOP revalidation.

Pre-Startup Safety Review

HAZOP conducted before plant startup identifies design deficiencies when changes are least costly — far cheaper than post-incident modifications or insurance claims from process accidents.

— OUR PROCESS —

HAZOP Study Process — Step by Step

Our 6-step HAZOP methodology follows IEC 61882, MSIHC Rules requirements, and DISH Gujarat documentation standards.

01
STEP 01

Pre-HAZOP Preparation

Collection and review of P&IDs, PFDs, process descriptions, operating procedures, MSDSs, equipment datasheets, and MSIHC consent conditions. Node division based on process sections, operating phases (startup, normal, shutdown, emergency), and utility systems.

02
STEP 02

HAZOP Team Formation

Formation of multi-disciplinary HAZOP team including process engineer, safety specialist, instrumentation engineer, operations representative, and maintenance lead. Prism's HAZOP facilitator leads the structured examination to ensure systematic coverage of all deviations.

03
STEP 03

Node-by-Node Examination

Systematic examination of each process node using standard guide words (No, Less, More, Reverse, Other Than, As Well As, Part Of) applied to process parameters (flow, pressure, temperature, composition, level) to identify all plausible deviations from design intent.

04
STEP 04

Cause-Consequence Analysis

For each identified deviation, the HAZOP team systematically identifies all plausible causes (equipment failure, human error, utility failure, external events) and consequences (fire, explosion, toxic release, environmental damage, business interruption). Risk ranking applied per client matrix.

05
STEP 05

Safeguard Assessment & Recommendations

Existing safeguards (IPLs — Independent Protection Layers) reviewed for adequacy against each identified scenario. Where risk exceeds acceptable level, actionable recommendations generated — design changes, additional instrumentation, procedural controls, or SIL-rated safety functions.

06
STEP 06

HAZOP Report & Action Register

Comprehensive HAZOP report in DISH Gujarat and regulatory-acceptable format: study methodology, node descriptions, worksheets with all deviations/causes/consequences/safeguards, recommendations, action register with responsible persons and target dates. Available for MSIHC renewal and EC compliance submissions.

What Our HAZOP Clients Say

Prism conducted HAZOP for all 12 nodes of our distillation unit. The team's process chemistry knowledge and PHA methodology were outstanding. The final HAZOP report with action tracker was comprehensive and accepted by DISH Gujarat without revision. Highly recommended for any MSIHC facility.
Ramesh Patel
For our EC renewal, MoEFCC required an updated HAZOP study. Prism's team understood pharmaceutical synthesis processes — their deviation analysis identified 3 critical safeguard gaps in our reactor system that we had missed. Their final report sailed through MoEFCC technical scrutiny.
Anita Mehta
We had a HAZOP conducted by Prism for our new chlor-alkali plant prior to commissioning. Their systematic node-by-node analysis identified issues with our brine recycle loop that the design team had not flagged. Worth every rupee spent before startup.
